WAIVER OF LIABILITY FORM

All students are required to fill out the waiver form below. Please read the information below carefully and fill out the form to accept the terms. If you have any questions, email info@bhangraclasses.com

I fully understand and agree to the policy and terms of Bhangra Empire's Bhangra Class.

I understand that:

  • Online classes take place virtually at the given class time
  • Live Class videos are recorded and will be made available after the class has taken place
  • Live class videos will be available for the duration of the entire class session
  • Class videos are for practice use only and should not be shared or distributed in any sort of way
  • There are no adjustments or refunds for missed classes
  • There will be no refunds unless canceled 2 weeks prior to start of class

Failure to comply with any of the above will result in immediate dismissal from the class.

I acknowledge, agree and understand that Bhangra is a physical activity that may be hazardous to some individuals, and may result in injury to myself or other dance students. I agree to assume all risks of injury incurred or suffered while attending bhangra classes, and release and agree not to sue Bhangra Empire, its instructors, or anyone connected with the bhangra classes for any claim, damages, costs or cause of action which I have or may have in the future as a result of injuries or damages sustained or incurred while attending classes.

COVID-19 Liability Release Waiver (for in person classes)

*I release Bay Area Bhangra Empire, LLC, it’s instructors, independent contractors, and all other associates from liability for harm, theft, or injury that may be suffered by me and/or members of my family traveling to or from or during participation in activities and programs sponsored by Bay Area Bhangra Empire, LLC. I hereby acknowledge that I am voluntarily assuming full responsibility for all risks of physical injury arising out of active participation in a dance class or other dance related activities. I acknowledge the contagious nature of COVID-19 and other contagious diseases and viruses and voluntarily assume the risk that I and/or my children may be exposed to or infected by COVID-19 by attending and participating and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 and other contagious diseases and viruses may result from the actions, omissions, or negligence of myself and others, including, but not limited to, employees, independent contractors, volunteers, and program participants and their families. I release Bay Area Bhangra Empire, LLC, its instructors, independent contractors and all associates from liability for harm, injury or death pertaining to COVID-19 and other contagious diseases and viruses.. I understand and agree to Bay Area Bhangra Empire, LLC's policies and procedures. I understand that I must have this release form signed before attending class. I give permission to Bay Area Bhangra Empire, LLC to use pictures and or video of my child for advertising purposes in print, promotional videos and on the website.
