WAIVER OF LIABILITY FORM

2026 Participant Agreement

Every participant must complete this waiver before taking part in a Bhangra Empire class, workshop, rehearsal, performance, private lesson, or related activity. A parent or legal guardian must complete it for anyone under 18.

01Assumption of Risk

I understand that dance and physical exercise involve inherent risks, including overexertion, muscle strains, sprains, falls, collisions, and other injuries. I voluntarily choose to participate and knowingly assume these inherent risks.

02Health and Safety

I confirm that the participant is physically able to participate. I am responsible for consulting a medical professional when appropriate, informing the instructor of relevant limitations, following safety instructions, using appropriate footwear and clothing, and stopping activity if the participant feels pain, dizziness, or illness.

03Release of Liability

In exchange for being permitted to participate, I release and agree not to sue Bay Area Bhangra Empire, LLC, Bhangra Empire, and their owners, instructors, employees, independent contractors, volunteers, agents, and venue partners for claims arising from the participant’s involvement in covered activities, including claims resulting from their ordinary negligence, to the fullest extent permitted by California law.

This release does not apply to gross negligence, recklessness, willful misconduct, or any liability that cannot legally be waived.

04Emergency Medical Care

If I cannot be reached in an emergency, I authorize Bhangra Empire and its representatives to contact emergency services and obtain reasonably necessary care for the participant. I understand that I am responsible for resulting medical and transportation costs.

05Conduct and Class Policies

The participant agrees to follow instructor directions, act safely and respectfully, and comply with the registration, attendance, cancellation, refund, recording, and content-sharing policies provided for the applicable program. Class recordings and choreography materials are for enrolled participants’ personal practice only and may not be shared or distributed without written permission.

06Photo and Video Authorization

I authorize Bay Area Bhangra Empire, LLC to photograph and record the participant and to use the participant’s image, likeness, voice, and performance for promotional and educational purposes in print, video, social media, advertising, and on its websites. This authorization is confirmed by a required checkbox in the form below.

07Minors

If the participant is under 18, I confirm that I am the participant’s parent or legal guardian, have authority to sign this waiver, consent to the participant’s involvement, and agree to these terms on the participant’s behalf.

08Acknowledgment and E-Signature

I have carefully read and understand this waiver. I sign it voluntarily and understand that I am giving up certain legal rights. If any provision is found unenforceable, the remaining provisions will continue in effect. My typed legal name, required acceptance checkbox, and electronic submission are intended to serve as my signature.
